Overview:

A failure cause is an indication of the deficiencies of the design/process/asset, whose consequences are the failure modes. It can be defined also as the way in which failure can occur, described for something that can be corrected or controlled. There is a direct relationship between the cause and its resulting failure mode, that is, if the cause occurs, then failure mode occurs. Identifying the root cause (or root causes) of a failure mode supplying enough details, allows for the identification of controls and appropriate action plans. Examples of causes for failure include: overloading, incorrect specification of the material, poor maintenance instructions, insufficient lubrication capacity, among others.

Click on the arrow next to this button to export the list of records displayed on the main screen to an Excel file, configure a new report or associate an existing report, or search for the parameters that may be used in the configuration of new reports.

Click on this button to enable/disable the record selected on the main screen. To view the disabled records, use the "Status" field of the General filter, in the search filters panel.

 

Use the "Search filters" to easily retrieve records based on specific information.
